INDICATION

Optibrex-D Eye Drop is a combination of an aminoglycoside antibiotic and a corticosteroid indicated for steroid-responsive inflammatory conditions of the eye where bacterial infection or risk of infection exists. It is prescribed for bacterial conjunctivitis with inflammation, blepharoconjunctivitis, keratitis, iridocyclitis, and post-operative inflammation after cataract surgery or other ocular procedures.

MECHANISM OF ACTION

Tobramycin provides bactericidal activity by binding to the 30S ribosomal subunit of susceptible bacteria, inhibiting protein synthesis. Dexamethasone is a potent synthetic corticosteroid that suppresses inflammation by inhibiting phospholipase A2, reducing prostaglandin, leukotriene, and cytokine production, stabilizing cellular membranes, and reducing vascular permeability. The combination addresses both the infection and the inflammatory component simultaneously.

DOSAGE AND ADMINISTRATION

     Instill 1 to 2 drops into the affected eye(s) every 4 to 6 hours

     For severe infections: instill every 2 hours initially, reducing frequency as the condition improves

     Do not exceed 14 days of treatment without ophthalmologist review

     Wash hands thoroughly before and after each application

     Space at least 5 minutes between other eye drop applications

SIDE EFFECTS

Common:

     Temporary blurred vision after instillation

     Mild burning or stinging

     Eye redness and mild irritation

Less common:

     Increased intraocular pressure with prolonged corticosteroid use

     Posterior subcapsular cataract formation with extended therapy

     Secondary infection due to corticosteroid-induced immunosuppression

     Allergic contact dermatitis of the eyelids

NOT RECOMMENDED IN

     Herpes simplex keratitis, vaccinia, varicella, and other viral eye infections

     Mycobacterial or fungal eye infections

     Known hypersensitivity to aminoglycosides, corticosteroids, or any component

DRUG INTERACTIONS

Concurrent use with other corticosteroid eye drops may increase the risk of elevated IOP. Avoid concurrent use with other aminoglycosides to prevent additive toxicity. Remove contact lenses before application.

STORAGE INSTRUCTIONS

Store below 25 degrees Celsius. Protect from light. Do not freeze. Discard 4 weeks after opening. Keep out of reach of children.
