INDICATION
Optinac Eye Drops 5ML is a non-steroidal anti-inflammatory ophthalmic suspension indicated for the treatment of pain and inflammation associated with cataract surgery and for reducing the risk of postoperative macular edema in diabetic patients undergoing cataract extraction. Nepafenac is a prodrug that penetrates the cornea rapidly and is converted to its active metabolite amfenac within the eye, providing targeted anti-inflammatory action.
MECHANISM OF ACTION
Nepafenac is an uncharged prodrug that crosses the intact cornea efficiently. Once inside the eye, it is hydrolyzed by intraocular tissue esterases to amfenac, a potent inhibitor of both COX-1 and COX-2 enzymes. Amfenac reduces prostaglandin synthesis at the site of ocular inflammation, decreasing pain, swelling, and the risk of cystoid macular edema. The prodrug design achieves higher intraocular drug concentrations with lower corneal surface toxicity.
DOSAGE AND ADMINISTRATION
● Instill 1 drop in the affected eye(s) 3 times daily starting 1 day before cataract surgery
● Continue on the day of surgery and for 2 weeks postoperatively
● Shake the bottle well before each use
● Do not wear contact lenses during treatment
● Space at least 5 minutes between other eye drop applications
SIDE EFFECTS
Common:
● Mild stinging and burning upon instillation
● Eye redness and irritation
● Blurred vision immediately after application
● Sticky sensation in the eye
Less common:
● Corneal epithelial defects and delayed wound healing
● Corneal thinning or perforation with prolonged use
● Allergic reactions including eyelid edema
● Increased intraocular pressure (rare)
NOT RECOMMENDED IN
● Known hypersensitivity to nepafenac or any NSAID
● Patients with complicated or prolonged corneal healing history
● Active corneal epithelial defect
● Use beyond 14 days without ophthalmologist review
DRUG INTERACTIONS
Concurrent use with topical corticosteroids may increase corneal adverse event risk. Topical NSAIDs may slow corneal healing. Avoid concurrent use with other ophthalmic NSAIDs.
STORAGE INSTRUCTIONS
Store between 2 to 25 degrees Celsius. Do not freeze. Shake well before use. Discard 4 weeks after opening. Keep out of reach of children.
